RN - Behavioral Health (CTI Team)
Key Responsibilities
- Assess and monitor participants' physical, cognitive, and psychosocial health.
- Support transitions from hospital/ED to community with care planning and follow-up.
- Provide health education, medication reconciliation, and disease prevention guidance.
- Link participants to community resources and healthcare providers.
- Collaborate with clinicians, care managers, and peer specialists to ensure continuity of care.
- Conduct field visits and provide flexible, community-based support.
Qualifications
- NYS RN license and registration required.
- Minimum 1 year of post-licensure nursing experience preferred, ideally with behavioral health populations or community/home visits background.
- Strong knowledge of health promotion, disease prevention, and community resources.
- Ability to work flexibly in field-based settings.

Pay Range: $90,000-$92,000 Per Year Plus an Additional $2k for Anyone Who is Bilingual Spanish
