Territory: Team 1 (northern) covers Maywood, Oak Park, Forest Park, Melrose Park, Elmwood Park, Franklin Park, Schiller Park, Schaumburg, Carol Stream, and Chicago zip codes north of North Ave.
About the Role
Our home care RNs use cutting-edge technology and clinical expertise to deliver one-to-one care and monitoring in patients’ homes. Responsibilities include client assessment, care planning, intervention, and supervision of patient outcomes while following physician orders and complying with the state’s Nurse Practice Act and all licensure requirements.
Responsibilities
- Provide one-to-one care to patients in their homes
- Conduct patient assessments and develop care plans
- Implement interventions and monitor outcomes
- Supervise patient care in compliance with physician orders and regulatory standards
Requirements
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Illinois
- 2–3 years of acute care nursing experience (home care preferred)

Pay Range: $36.70–$58.71 per hour
