About the role

• Provide one-on-one skilled nursing care to pediatric and adult clients with chronic, complex, or medically fragile conditions in their homes.
• Collaborate with physicians, therapists, case managers, and family members to support the client’s individualized plan of care.
• Deliver continuous hourly care, ensuring safety, comfort, and clinical stability throughout each shift.

Requirements

• Valid Indiana nursing license in good standing
• Minimum of one year of nursing experience as a Registered Nurse (RN)
• Current CPR certification (Free re-certification with Adaptive)
• Good organizational and communication skills
• Compliance with state background screening requirements
