RN Ambulatory - Full-Time - PACE Elder Service Plan
About the role
The Registered Nurse is a crucial member of the PACE Interdisciplinary Team (IDT), providing PACE program participants with coordinated nursing care in their own homes. The RN demonstrates expertise in the assessment, case management, and implementation of participants’ overall plans of care. They also utilize evidence-based nursing interventions to support these comprehensive care plans, as well as collaborate with the enrollment team in performing initial assessments of participants’ home care needs, such as medication reconciliation and warm hand-offs to other IDT members. In addition, the RN communicates the current status of participants to the IDT, including new problems and/or problems indicating a significant change in status, in order to inform best practices for continued care.
Responsibilities
- Provide coordinated nursing care to PACE program participants in their homes
- Assess and implement participants' overall plans of care using evidence-based nursing interventions
- Collaborate with the enrollment team in performing initial assessments of participants’ home care needs, such as medication reconciliation and warm hand-offs to other IDT members
- Communicate the current status of participants to the IDT, including new problems and/or problems indicating a significant change in status, to inform best practices for continued care
Qualifications
- Current or conditional Massachusetts RN licensure required
- American Heart Association BLS Provider certification required; Phlebotomy skills preferred
- B.S. in Nursing preferred
- One to three years of home care experience preferred
- Highly developed geriatric assessment and clinical skills
- Experience at a PACE plan, working on an interdisciplinary team, etc. is desired
- Fluency in Spanish, Haitian Creole, Portuguese, etc. is desired
- Clinical experience with geriatric, behavioral health and/or disabled populations is desired
Pay
The final offer may vary within the listed Pay Range, based on a candidate's experience, skills, qualifications, and internal equity considerations.
Schedule
Day, Monday - Friday, 8:00am with varied end times, Hours/Week: 36.00, Every other weekend, Rotating holidays, None.
