The professional nurse at Jefferson Health Primary Care uses the nursing process to meet the diverse needs of patients, collaborating with providers and support staff.
Responsibilities
- Collaborate with providers and support staff to deliver clinical nursing interventions.
- Communicate effectively with patients, respecting their cultural beliefs and values.
- Maintain a current nursing license and integrate evidence-based practice in patient care.
- Support efficient communication between primary care providers and specialists.
- Participate in quality improvement activities and clinical care pathways.
- Provide patient education on preventive care, chronic disease management, and medication management.
- Document all patient interactions and clinical care in the EMR.
- Perform care coordination tasks such as managing in-baskets and abnormal results.
- Assess, promote, maintain, and restore patients' health and wellness.
- Minimize risks to patients by adhering to established nursing practices and protocols.
- Use information technology to communicate and manage knowledge.
- Monitor and improve care processes using data and improvement methods.
- Work effectively within primary care inter-professional teams.
- Provide safe, compassionate, and coordinated care based on patient preferences and values.
- Create an inclusive environment and demonstrate problem-solving and conflict resolution skills.
- Participate in team meetings and provide updates.
- Seek opportunities for professional growth and development.
Requirements
- Current RN licensure in state of practice.
- BLS certification based on organizational guidelines/policies.
Qualifications
- Graduate of an accredited school of nursing.
- Bachelor of Science in Nursing (BSN) or completion of BSN or appropriate advanced degree within 3 years of hire.
- 1+ years’ experience as a Nurse in an ambulatory setting preferred.
