About the role

The professional nurse at Jefferson Health Primary Care uses the nursing process to meet the diverse needs of patients, collaborating with providers and support staff.

Responsibilities

• Collaborate with providers and support staff to deliver clinical nursing interventions.
• Communicate effectively with patients, respecting their cultural beliefs and values.
• Maintain a current nursing license and integrate evidence-based practice in patient care.
• Support efficient communication between primary care providers and specialists.
• Participate in quality improvement activities and clinical care pathways.
• Provide patient education on preventive care, chronic disease management, and medication management.
• Document all patient interactions and clinical care in the EMR.
• Perform care coordination tasks such as managing in-baskets and abnormal results.
• Assess, promote, maintain, and restore patients' health and wellness.
• Minimize risks to patients by adhering to established nursing practices and protocols.
• Use information technology to communicate and manage knowledge.
• Monitor and improve care processes using data and improvement methods.
• Work effectively within primary care inter-professional teams.
• Provide safe, compassionate, and coordinated care based on patient preferences and values.
• Create an inclusive environment and demonstrate problem-solving and conflict resolution skills.
• Participate in team meetings and provide updates.
• Seek opportunities for professional growth and development.

Requirements

• Current RN licensure in state of practice.
• BLS certification based on organizational guidelines/policies.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor of Science in Nursing (BSN) or completion of BSN or appropriate advanced degree within 3 years of hire.
• 1+ years’ experience as a Nurse in an ambulatory setting preferred.
