RN, Admissions Registered Nurse

Responsibilities

Conduct timely and thorough assessments to ensure a smooth hospice admission.

Educate patients and families on hospice services, symptom management, and next steps.

Serve as a liaison between hospice, hospital teams, and healthcare providers, ensuring clear communication.

Advocate for patients and families, addressing concerns with compassion and clinical expertise.

Qualifications

• Associates, Bachelors or Masters Degree in Nursing accepted
• Active unencumbered Registered Nurse license
• 1+ years of nursing experience with exposure to hospice patients
• Reliable transportation, driver’s license, and auto liability coverage
• CPR certification, TB test results, and an updated physical
