RN - Admissions/Liaison

About the Role

The Admission/Liaison Nurse is an integral part of the Interdisciplinary Team and works closely with the Medical Director for determining and facilitating hospice admissions. This role is primarily responsible for coordination and execution of the admission process into Hospice care.

You will serve patients and families in NE Oklahoma and NWA, effectively communicate the Hospice concept to families and patients, develop the plan of care, and communicate patient/family care needs to the team. You will train and educate patients/caregivers and establish an appropriate frequency of disciplines to meet patient/family needs. Flexibility to change your schedule at a moment's notice to meet patient needs is required.

Daily travel is required, and you will manage your own schedule effectively. You must have excellent clinical skills to identify problems, perform head-to-toe assessments, educate and consult with physicians, and execute orders to meet patient needs.

Responsibilities

• Coordinate and execute the admission process into Hospice care
• Communicate the Hospice concept to families and patients
• Develop the plan of care and communicate patient/family care needs to the team
• Train and educate patients and caregivers
• Establish an appropriate frequency of disciplines to meet patient/family needs
• Perform head-to-toe assessments and identify clinical problems
• Educate and consult with physicians, and execute orders to meet patient needs
• Manage your own schedule and travel daily to meet patient needs
• Cope with human suffering while maintaining professional boundaries

Requirements

• Current unrestricted AR RN license
• BLS certification
• Strong assessment skills with critical thinking and problem-solving abilities
• Ability to stay calm and cope with human suffering and emergencies
• Strong documentation and communication skills
• Use of personal vehicle, valid driver's license, and current auto insurance

Qualifications

• Hospice experience (preferred)
• Home health or acute care nursing experience (preferred)
• Computer charting experience (preferred)

Schedule

A variety of shift configurations are available, including nights, days, and weekends.
