Job Summary
These functions/duties include but are not necessarily limited to the following:
- explains hospice services to patients and families and assist with the signing of consent documents.
- Collaborate and develop plan of care with patient, significant others, and members of the interdisciplinary team to focus and assure patient centered outcomes.
- Assesses, plans, implements, documents, organizes, prioritizes, teaches, delegates, supervises, and coordinates the care of adult and geriatric patients; (initial assessment, planning, coordination, evaluation, and teaching)
- Provides care and support to terminally ill patients and their families;
- Perform other nursing duties as requested.
Requirements
- Education and Experience: Graduate of an accredited School of Nursing
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
- One year of experience in an acute, hospice, home health setting, or long-term care setting.
- Bachelor's degree in nursing a plus.
- Two years of experience a plus.
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
Other
- Valid state-issued driver's license required.
- Must carry automobile liability insurance at limits required by agency.
- Must have own transportation.
