Job Summary

These functions/duties include but are not necessarily limited to the following:

• explains hospice services to patients and families and assist with the signing of consent documents.
• Collaborate and develop plan of care with patient, significant others, and members of the interdisciplinary team to focus and assure patient centered outcomes.
• Assesses, plans, implements, documents, organizes, prioritizes, teaches, delegates, supervises, and coordinates the care of adult and geriatric patients; (initial assessment, planning, coordination, evaluation, and teaching)
• Provides care and support to terminally ill patients and their families;
• Perform other nursing duties as requested.

Requirements

• Education and Experience: Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• One year of experience in an acute, hospice, home health setting, or long-term care setting.
• Bachelor's degree in nursing a plus.
• Two years of experience a plus.
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Other

• Valid state-issued driver's license required.
• Must carry automobile liability insurance at limits required by agency.
• Must have own transportation.
