RN, Admissions 10a-8p every other weekend

About the role

The Admission RN plays a crucial role in Chapters Health System, ensuring a seamless transition for patients into the organization's services. This position involves assessing patients, initiating care, and communicating effectively with patients, families, and healthcare providers.

Responsibilities

• Completes clinical admissions for all Company services, explaining services and addressing patient concerns.
• Identifies admission objections and provides responses, educating patients on admission requirements.
• Completes informed consents and initial assessments, initiating Plan of Care and obtaining physician orders.
• Documents care according to Company policy, communicating with physicians and Care Navigation staff.
• Facilitates referrals to all service lines, managing patient discharges efficiently.
• Works with PRRs to educate on available services, managing GIP patients, and responding to emergencies.
• Performs other duties as assigned, adhering to physical demands.

Qualifications

• Current RN license in the state where working.
• One (1) year nursing experience, preferably in hospice or hospital settings.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills, particularly in emergency departments or ICUs.
• Bilingual in Spanish preferred.
• Effective verbal and written communication skills.
• Valid driver’s license and auto insurance.
• Ability to manage multiple priorities and handle emotional stress.
• Physical ability to perform essential job functions, including lifting and moving patients.
