About the Role
SUNY Upstate is seeking a highly motivated Retention and Adherence LPN Specialist to provide services to patients in the Inclusive Health Services and the Pediatric Infectious Disease Clinic's grant-funded retention and adherence program. The role focuses on linkage to and retention in medical care, as well as Intensive Case Management for patients new to antiretroviral therapy and/or those who have not achieved viral suppression.
Responsibilities
- Assist patients in identifying barriers preventing engagement in medical care and attainment of a suppressed HIV viral load.
- Collaborate with patients to create plans addressing these barriers.
- Encourage and follow up with patients to help them reach their goals.
- Improve health outcomes for patients and reduce community viral load through prevention for positives.
- Demonstrate professionalism, leadership, and commitment to Upstate’s mission, vision, and values while adhering to institutional policies and procedures.
- Work as an integral member of an interdisciplinary healthcare team.
Requirements
- Current NYS LPN license.
- Minimum of 5 years of experience in Health and Human Services.
- CPR Certification required at the time of appointment.
- Case Management Experience.
- Experience in the field of HIV/AIDS.

- Strong interpersonal, written, and oral communication skills.
- Ability to work effectively in an interdisciplinary team environment.
Schedule
Monday–Friday, 8:00 AM–4:30 PM.
Pay
Salary range: $40,000 – $50,000, dependent on experience.
