This position is full-time on site at our The Dalles Clinic location in Hood River, Oregon.
About the role
This patient-focused role operates the 340B Retail Pharmacy within the One Community Health clinic. The Retail Pharmacist provides high-quality pharmaceutical care, ensures the safe and effective use of medications, and collaborates with a multidisciplinary healthcare team.
Responsibilities
- Accurately dispense medications prescribed by clinic providers, ensuring proper labeling, packaging, and dosage instructions.
- Provide comprehensive counseling to patients on medication use, including potential side effects, interactions, and adherence strategies.
- Conduct medication reviews, collaborate with patients and providers to optimize therapy, and address medication adherence issues (Medication Therapy Management - MTM).
- Partner with healthcare providers and the clinical support team to deliver optimal pharmaceutical care, including offering therapeutic recommendations and drug information.
- Support patients with chronic conditions (e.g., diabetes, hypertension, asthma) through medication management, follow-ups, and education on lifestyle modifications.
- Administer immunizations in accordance with state regulations and clinic protocols.
- Educate patients on disease prevention, health promotion, and lifestyle changes to enhance medication effectiveness.
- Oversee medication and supply inventory, ensuring accurate stock levels, ordering, and storage in compliance with regulations.
- Ensure pharmacy operations comply with federal, state, and clinic standards. Monitor pharmacy performance metrics and implement strategies to optimize 340B program benefits and expansion.
- Uphold and promote One Community Health’s philosophy, mission, and core values. Consistently follow and support established guidelines and procedures.
Requirements
- Doctor of Pharmacy (PharmD) from an accredited college of pharmacy.
- Active and unrestricted Oregon Pharmacist License (or ability to obtain within 45 days of hire).
- At least one year of experience in a retail pharmacy setting.
- Prior experience with the 340B Drug Pricing Program.
- Comprehensive clinical knowledge and attention to detail.
- Strong passion for patient education and medication management.
- Proficiency in pharmacy management software systems.
- Excellent communication, problem-solving, and mentorship skills.
- Ability to work both independently and collaboratively within a diverse healthcare team.

Preferred Qualifications
- Bilingual (English/Spanish).
- Active pharmacist immunization certification.
- Experience in chronic disease management and Medication Therapy Management (MTM).
- Previous experience in a community or migrant health center (FQHC) or similar healthcare setting.
- Proficiency with EPIC Willow Ambulatory or similar pharmacy management systems.
- Board of Pharmaceutical Specialties Certification (BCACP or BCPS).

Pay
Salaried/Exempt: $150,000.00. Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
