About the Role

The Redding Rancheria is seeking a Resource Care Coordinator to assist with the day-to-day coordination and operation of the medical department. This is a float position responsible for providing relief coverage and assistance to balance workflows of the Care Coordination team, working in cooperation with medical providers to ensure quality patient care and timely address of patient needs.

Requirements/Minimum Qualifications

  • Graduate of an accredited school of professional nursing and licensed to practice as a nurse (LVN) in the state of California.
  • 2+ years of clinic nursing experience preferred.
  • Experience with EHR (NextGen) strongly desired.
  • CPR certification.
  • Must pass pre-employment drug screening.
  • Indian preference.
  • Must be fingerprinted and pass a background check.

Essential Job Duties

  • Demonstrates competency and professional responsibility in the Care Coordinator role, following provider orders, evaluating nursing interventions, implementing emergency procedures, and complying with clinic policy and scope of practice.
  • Performs direct patient care, including rooming patients, obtaining vital signs, documenting intake notes, evaluating patient signs/symptoms, and reporting findings to appropriate personnel.
  • Appropriately documents in the electronic healthcare record (EHR) full vital signs, medication profile updates, immunization records, chief complaints, interventions provided, and responses.
  • Assists with daily workflow, including reviewing appointments, assisting with floor assignments, ensuring patient flow, and following up on quality improvement measures.
  • Assists with the maintenance and inventory of medical equipment, supplies, and educational materials, including participation in monthly medication counts and ensuring accurate temperature checks.
  • Provides patient education on symptom management, treatment plans, and pertinent health-related information within the scope of practice.
  • Effectively communicates relevant information, including changes in patient status and process improvement feedback, and maintains open communication with patients and staff.
  • Provides observational feedback of Medical Assistants to support performance evaluations and corrective measures.
  • Assists with Care Coordinator tasks, including patient referrals, provider PAQs, and rendering assistance to clinic providers within the scope of practice.
  • Assists with patient complaints regarding the Medical Department in real-time as needed.
  • Works on assigned projects/tasks under the direction of the Director of Nursing.

Schedule

The preferred schedule for this position is Monday - Friday, 8:00 AM - 5:00 PM.
