Residential LPN - All Shifts Available

About the role

LifeStream, Inc. is now hiring full-time and part-time Licensed Practical Nurses (LPNs) to support the medical and daily living needs of individuals with disabilities.

Responsibilities

• Assess all individual's medical regimes to ensure health services are provided.
• Ensure all consumers are referred for and receive services that assist with achieving set goals and maximize physical and emotional well-being.
• Provide direct care duties to maintain high levels of care standards.
• Maintain medical charts, medication ordering regimes, and other medical supply inventory.
• Assess all consumer complaints and overall well-being of each individual on a daily basis.
• Review doctor’s orders for accuracy.
• Adhere to DPH regulations for medication storage, administration, and destruction.

Requirements

• Maintenance of an acceptable CORI and fingerprinting.
• Valid Driver’s license.
• Negative drug screen.
• Use of personal vehicle for work purposes.
• Commitment to the philosophy of community inclusion.

Qualifications

• Licensed Practical Nurse with current licensure in the Commonwealth of Massachusetts.
• Demonstrated history of quality work performance and teamwork in previous positions.
• Familiarity with the needs of individuals with developmental disabilities and brain injury.

Pay

Starting pay is $34/hour, with weekend shift differentials available.
