About the Role

Assists individuals with daily personal routines, including providing prompts for daily living skills, skill-building activities, and participation in groups. Provides companionship to residents and establishes trusting, professional relationships. Works with licensed or visiting nursing personnel to monitor medical concerns and observe interaction with medications. Ensures living spaces are safe, sanitary, and well-organized. Leads group activities and outings. Collaborates with nursing, management, and coworkers to ensure residents receive the best quality services.

Responsibilities

  • Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhance clients’ independent living skills through engagement, assessment, and teaching (e.g., modeling, prompting, hands-on assistance).
  • Provide hands-on support for activities of daily living, including personal care/hygiene, housekeeping, laundry, food management/nutrition, budgeting, health and safety, and leisure activities.
  • Facilitate or co-facilitate psychoeducational groups as indicated.
  • Engage clients in community integration and social recreation activities.
  • Provide transportation to and from appointments as identified.
  • Educate and train clients on travel planning and use of public/private transportation.
  • Conduct regular apartment/room inspections and assist clients in maintaining cleanliness, including hands-on assistance if needed.
  • Perform ongoing safety rounds to ensure the well-being of individuals and the program.
  • Administer breathalyzer tests and urinalysis samples, as applicable.
  • Document shift logs in electronic records according to contract and agency policy.
  • Develop and maintain working relationships with providers, families, and natural supports.
  • Respond to crisis situations and provide appropriate interventions as outlined in agency policy for Nonviolent Practices.
  • Supervise, monitor, and document the self-administration of client medications.
  • Attend and participate in monthly staff meetings, clinical rounds, AllTreater meetings, and supervision.
  • Maintain a clean, well-organized work environment and ensure program egresses and walkways are clear for safety.
  • Notify supervisor of maintenance and/or safety issues and generate work orders as needed.
  • Conduct and participate in fire and emergency evacuation drills and review the emergency preparedness plan.
  • Participate in Food Bank activities as assigned.
  • Remain awake during all shifts to maintain the security and safety of clients and the facility.
  • Adhere to policies and practices outlined in the staff handbook and program operations manual.
  • Uphold HIPAA regulations and secure necessary releases for outside communication.
  • Model culturally competent behavior when interacting with clients, providers, family, and staff.
  • Maintain accurate electronic records and case files in accordance with programming, contractual, licensing, and accreditation requirements.
  • Adhere to CARF (Council on the Accreditation of Rehabilitation Facilities) standards.
  • Participate in agency committee work when available or permitted.
  • Complete all required trainings per program contract and agency policy.
  • Attend agency-sponsored in-services, outside trainings, conferences, and meetings to facilitate professional growth.

Requirements

  • High School Diploma or equivalent, with a minimum of one (1) year of experience in a related healthcare field.
  • Licensed CNA or other related healthcare certification accepted in lieu of experience.
  • Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction.
  • Knowledge of the community support system and resources preferred.
  • Valid Connecticut Driver’s license with a clean driving record is required.
  • Scattered site and non-24-hour program positions require reliable transportation and willingness to transport clients in personal vehicles.
  • CPR/First Aid certification (offered as part of orientation; employee responsible for maintaining ongoing certification).
  • Proficient computer skills, including Microsoft Office and electronic record keeping.

Pay

Starting pay: $21.46/hour with a weekend shift differential of $1 more per hour ($22.46/hour).
