Lifeworks is a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We are seeking an enthusiastic Teacher/Counselor (Residential Counselor) to support individuals with intellectual and developmental disabilities (ID/DD) in one of our residential programs.

Responsibilities

  • Promote optimal health for individuals through routine, preventive, and emergency health care.
  • Assist with all hands-on direct care, including but not limited to: showering, toileting, shaving, brushing teeth, and meal-time assistance.
  • Ensure residential environments are safe, secure, and free from harm.
  • Provide education, training, counseling, and physical assistance to achieve ISP (Individual Service Plan) goals in areas such as functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
  • Develop, write, and present annual ISPs and semi-annual assessments in consultation with other residential staff and directors.
  • Provide transportation to individuals as necessary.
  • Communicate with persons significant to the individual served (e.g., family, friends, guardians, funding sources, advocates, volunteers, employers) to facilitate consistency of programming and support.
  • Perform case management duties, including maintenance of individual confidential files, ensuring case reviews, documentation of activities, and training.
  • Attend regular meetings as necessary.

Requirements

  • Experience working with individuals with ID/DD.
  • A Bachelor’s degree or an associate degree in a related field with two years of experience, or a high school diploma and five years of related work experience in human services.
  • Must pass fingerprint background check, CORI, DPPC, and driving background checks.
  • Valid Driver’s License required.
  • Valid MAP certification is preferred.
  • Excellent verbal and written communication skills.

Schedule

  • Thursday: 1:00 PM – 9:00 AM
  • Friday: 12:00 PM – 10:00 PM
  • Saturday: 11:00 AM – 9:00 AM
  • Sunday: 9:00 AM – 1:00 PM

Pay

$69,888.00 per year
