About the role

We're seeking 1 full-time staff member who can work days, evenings, or nights. Some shifts include a shift differential of $2 for evenings, $4 for nights, and $4 for weekends, which can be stacked.

Responsibilities

  • Facilitate interventions related to independent living skills for residents
  • Affords patients and their engagement with support systems to enhance recovery efforts (e.g., family, friends)
  • Aid in coordinating specialty referrals (e.g., cardiologist, dentist) and communication with primary care providers
  • Aid in keeping medication sheets up to date (confirm signatures, staff initials, correct/note med variances)
  • Provide direct client care by implementing programs and facilitating and monitoring groups
  • Ensure a safe, comfortable, and therapeutic environment for consumers and staff
  • Become familiar with each consumer's treatment plan relating to placement, interventions, and goals and discharge plans
  • Plan, implement, and evaluate services to the consumers
  • Prepare and maintain paperwork and other documentation (charting, daily report sheets, etc.)
  • Plan and/or participate in appropriate recreational activities to promote consumers' well-being
  • Provide transportation for residents to meet requirements or activities outlined in their overall treatment plans
  • Maintain confidentiality of all consumers
  • Complete electronic medical record documentation as required
  • Promote a therapeutically healthy environment for all consumers
  • Participate in team programs, staffings, meetings, and special projects
  • Develop and revise treatment plans as needed
  • Enforce rules and regulations for appropriate conduct of consumers and visitors
  • Transport clients when necessary
  • Act as a liaison to community and governmental agencies
  • Comply with policies and procedures related to the care and well-being of all consumers, ensuring respect and dignity in treatment
  • Perform other duties as assigned

Requirements

Associates' or Bachelor’s Degree in Social Work, Psychology, or related human services discipline is required. Experience working with program population and program environment is preferred. Must possess a current valid driver’s license to operate an automobile.

Knowledge of social work principles, psychology, and human services. Ability to facilitate interventions related to independent living skills. Proficiency in managing patient care and coordination of specialty referrals. Strong organizational and communication skills. Pay

Starting pay from $18.75 and up based on experience.

Schedule

Shifts can be days, evenings, or nights, with the option to stack shift differentials for weekends and evenings.
