About the program
Oak Street 10 Bed Co-Ed Transitional Living Program for adults with Substance Use Disorders in Willimantic. Clients must attend outpatient treatment while in the program. Clients must have a discharge plan as it is a transitional living.
Duties & responsibilities
- Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
- Makes sure all residents take their medications according to the Adult residential medication policy and procedure
- Communicates effectively with other staff about the day’s events
- Teaches and helps residents with rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills
- Helps manage administrative tasks as directed by the supervisor
- Organizes and leads educational and skill-building groups and one-on-one interventions as per the treatment plan
- Collaborates and coordinates care with providers and families as needed
- Supports residents in dealing with medical and psychiatric needs
- Documents all activities in compliance with agency and program standards
- Attends staff meetings, supervision, trainings, and other meetings as assigned
- Performs additional duties as required
Qualifications
- High School Diploma or equivalent
- Experience in Human Services with a responsible work history
- Valid driver’s license required with a good driving record
- CPR & First Aid Certification required OR must obtain within 90 days of employment
