What You'll Do
- Report to the Program Director and Nurse Coordinators.
- Work as part of a team in a residential program for fragile elderly residents to provide practical support and assistance with activities of daily living as well as emotional and social support to program residents.
- Avoid assisting program case managers and program coordinator.
- Make regular contact with residents to ensure safety and well-being and to monitor needs for support.
- Become and remain fully familiarized with each resident's situation and needs through review of intake documents, daily log, case management notes, staff meetings and interactions with residents.
- Accurately record in daily logbook all activities performed with each resident, including personal care and practical support provided.
- Perform other duties as requested by supervisor.

Requirements
- High school diploma or GED required; some college preferred.
- Certified Nurse’s Aide or Home Health certification preferred.
- Experience in any of the following areas desirable: Special needs residential work, hospice, and home health care, experience working with the elderly, substance abuse or mental health treatment.
- Excellent oral, interpersonal and written communication skills.
- Demonstrated awareness of and sensitivity to issues related to the elderly, mental illness, racial/ethnic diversity, and addiction.
- Energetic and self-motivated reliability and flexibility.
- Demonstrated knowledge of and sensitivity to issues related to HIV, substance abuse, poverty & homelessness, and other populations required.
- Background and driving record checks will be performed.
