Resident Services Coordinator/Social Servies Coordinator

About the role

Our Resident Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implementing and evaluating the Social Services program to meet the psycho-social needs of residents.

Responsibilities

• Assessing resident needs based on admission social services history, resident’s health, psychological and social needs
• Maintaining resident records
• Linking residents to specific supportive services needed to continue living independently
• Promoting a positive living experience for residents
• Ensuring confidentiality by adhering to HIPAA and all other privacy standards
• Promoting a positive work environment by collaborating with community team members

Requirements

• 1+ years of experience in case management (required)
• 2+ years of social services experience, preferably in a senior care setting
• Bachelor’s Degree required in social work or related program such as gerontology, psychology, or recreation therapy
• Bilingual – English/Mandarin
• Social Services certification preferred

Pay

Pay Range: $25.00 - $27.00 per hour, depending on experience.

Schedule

FULL-TIME Monday - Friday, 9:00 AM – 5:00 PM.
