Resident Services Coordinator/Social Servies Coordinator

About the role

Our Resident Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implementing and evaluating the Social Services program to meet the psycho‑social needs of residents. This may include resident assessment, maintenance of resident records, and ensuring residents are linked to the specific supportive services they need to continue living independently in the community.

Responsibilities

• Assess resident needs based on admission social services history, health, psychological and social needs
• Maintain resident records
• Link residents to appropriate supportive services
• Promote a positive living experience for residents
• Ensure confidentiality by adhering to HIPAA and other privacy standards
• Collaborate with community team members to promote a positive work environment

Requirements

• 1+ years of experience in case management (required)
• 2+ years of social services experience, preferably in a senior care setting
• Bachelor’s Degree in social work or a related field such as gerontology, psychology, or recreation therapy
• Bilingual – English/Mandarin
• Social Services certification preferred

Pay

Pay Range: $25.00 – $27.00 per hour, depending on experience

Schedule: FULL‑TIME Monday – Friday, 9:00 AM – 5:00 PM
