JOB SUMMARY
Assists with Coordination of medical appointments to meet resident needs, drive wheelchair van/accompany residents to appointments to maintain continuity of care and safety.
ESSENTIAL FUNCTIONS
CARE GIVING
- Schedules both in-house and external medical appointments to maintain continuity of care and resident safety.
- Drives/Accompanies residents to medical appointments.
- Maintains and updates resident physician appointment schedules and communicates same to all appropriate staff.
COMPASSIONATE ATTITUDE
- Treats resident as individual, being polite, sharing positive experiences, listening to resident, allowing resident enough time to respond.
- Gives resident choices where applicable, helping the resident to understand consequences of choices offered.
SAFETY PROTOCOLS
- Observes established safety protocols.
- Removes safety hazards immediately and notifies other departments as appropriate.
- Maintains residents in appropriate wheelchairs during transportation and ensures they are not left unattended.
- Affords proper documentation in case of problems during transportation.
- Transfers residents using proper tools and techniques.
COMMUNITY OF CARE
- Maintains awareness of preferences of the population served and communicates same to physicians and medical office staff.
- Supports the interdisciplinary team and contributes to team efforts.
- Notifies licensed staff of changes in resident care needs, emergency situations, or unusual changes noted.
- Charts resident information neatly and timely.
- Assures all paperwork needed for consults is completed in appropriate time frame.
- Participates in committees, projects, and performance improvement activities.
- Assists Unit Secretary with inventory of supplies, clerical duties, and cleanliness of the unit as needed.
- Maintains all aspects of HIPAA requirements.
PERSONAL CARE
- Assists resident with ADL skills and grooming, hygiene needs when necessary.
- Ensures residents are appropriately attired for outside appointments.
INFECTION CONTROL
- Ensures proper infection control standards and guidelines including but not limited to hand washing, glove use, and maintaining aseptic technique in the clinical environment.
- Complies with all equipment infection control maintenance standards.
DOCUMENTATION
- Ensures proper documentation of necessary laboratory forms and specimens attained by physicians and ensures appropriate transport of same in required time frame.
TECHNICAL KNOWLEDGE
- Maintains and develops new computer knowledge to meet department, organizational, and community needs.
Educational Requirements:
- High School Diploma required.
Experience Requirements:
- Previous LTC experience preferred.
Licenses / Certifications:
- Certified Nursing Assistant required.
Skills and Abilities:
- Demonstrated self-confidence, initiative, organization and managerial skills.
- Must be able to communicate effectively.
- Self-motivated.
