Resident Care Coordinator - Personal Care - LPN License Required

About the role

In this role you will be responsible for day-to-day operations as they relate to resident care while promoting and integrating the mission and philosophy of Senior Lifestyle Corporation. The RCC manages the care staff in AL by scheduling, evaluating, and supporting the frontline care staff while reporting to the HWD. The RCC should be in close contact with resident families and be instrumental in care plan meetings and service issues as they relate to care.

Responsibilities

• Support HWD by managing Assisted Living team members
• Schedule Assisted Living team members
• Participate in Care Plan meetings with families and residents and manage ISPs as resident needs change
• Promote all company policies by ensuring SLC communities are providing the best care possible for our residents
• Participate in weekly at-risk meetings with HWD, ED, MCC/MCD to ensure leadership team is aware of all resident needs
• Review and implement all necessary processes for optimal operational efficiency such as shower schedules, laundry schedules, caregiver assignment sheets, etc.

Qualifications

• Level of Formal Education: HS or GED
• Area of Study: General
• Years of Experience: 3–5+ years caregiver experience preferred
• Type of Experience: CNA experience preferred, scheduling, communicating with families
• Special Certifications: LPN
• Language Skills: Acceptable fluency in English to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties
• Technical Competencies: Basic computer skills, Excel for scheduling
