Resident Care Coordinator LPN
Job Summary
A Resident Care Coordinator LPN will work closely with the Health Services team in our Community Based Care facility to ensure the quality care of all residents. This position plays an important part of organizing and assisting with training new care staff and providing ongoing mentorship to our current employees.
Responsibilities
- Covers onboarding and orientation for new employees
- Acts as lead trainer for care team (including Caregivers and Med Techs)
- Oversees on-going training for all staff
- Reviews assignments with Executive Director and Director of Health Services
- Works with residents on activities of daily living while encouraging self-care and independence
- Affords leadership with putting together staff schedules
- Documents any resident change of condition
- Willing to cover shifts when needed
- Performs other duties as assigned
Requirements
- Licensed Practical Nurse (LPN) license in state of practice
