Key Responsibilities
- Captures care plan conferences with each resident and responsible family members, sending letters out on a monthly basis, and then following up with resident’s family members.
- Maintains current tasks lists for resident aides, and updates timely as ADL changes occur.
- Schedules the service plan dates according to state regulation guidelines.
- Familiar with the service plan program and all of the reports it creates.
- Implements interim service plans as care needs change and ensures all medication techs are properly trained on implementing interim service plans.
- Makes sure the medication room is properly stocked, organized, and clean.
- Performs department audits as directed by Health Services Director/Administrator.
- Assists Health Services Director with tracking and recording incident reports and ensures incident investigations are completed timely and thoroughly.
- In collaboration with the Administrator/Health Services Director/Partner Services Manager, hires, trains, supervises, counsels, and terminates staff as needed.
- Fills-in as needed and directed by the Administrator for receptionist, medication tech, and/or resident aide.
Minimum Education Required
High school graduate or equivalent
Minimum Experience Required
Minimum of four (4) years’ experience in long term or assisted living elder care.
A minimum of two (2) years’ experience in a supervisory role within the healthcare industry, with scheduling experience a necessity.
Minimum Licensure/Certification Required
By Law Current and unexpired CPR certification

- Previous supervisory experience in long term or assisted living elder care preferred.
- Degree in nursing from accredited school of nursing preferred.
