Resident Care Associate

About the role

The Resident Care Associate is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in assisted living and Memory care neighborhoods. This role includes providing hands-on care, physical and emotional support as outlined in each resident’s Personalized Service Plan (PSP) while maintaining a safe, comfortable home-like environment. The Resident Care Associate demonstrates all elements of wellness, adheres to federal, state, and local standards, and follows Phoenix policies to promote quality care and services.

Responsibilities

• Participates in the development and updates of Personalized Service Plans; attends service plan review meetings with families as requested.
• Ensures the hydration schedule established for residents is maintained during the shift.
• Completes resident meal attendance sheets and ensures any resident who has not dined in the dining room is provided room service or accounted for.
• Knows and complies with all resident rights.
• Initiates, assigns, and assists with activities as appropriate.
• Checks assigned residents daily for cleanliness, appropriate clothing, continence issues, and changes in condition.
• Uses proper sanitary procedures and universal precautions.
• Checks assigned resident rooms daily for general cleanliness and neatness; ensures high-risk rooms are double-checked.
• Assists with conducting laundry rooms and common areas (including bathrooms) checks for cleanliness and exposed hazardous chemicals; takes appropriate action (e.g., contacting housekeeping, personally cleaning, securing chemicals).
• Completes all Activities of Daily Living (ADLs) for assigned residents according to the resident’s Personalized Service Plan (PSP).
• Responds to security systems and resident call bells promptly and immediately; takes appropriate action including resetting call bells.

Requirements

• High school education preferred and may be required by the state.
• Previous experience working with elders or disabled individuals preferred.
• Must be at least 18 years of age to perform the personal care aspects of the role.
• Knowledge and experience in the Assisted Living industry and Dementia care or Skilled Nursing preferred.
• Ability to work weekends, evenings, and flexible hours, available during peak service delivery times.

Schedule

1st, 2nd, and 3rd shifts available Monday through Friday; every other weekend required.
