Qualifications
- Active Registered Nurse (RN) license in the State of Georgia.
- Hospice Experience Preferred.
- Minimum of 3 years of nursing experience required.
- Previous Supervisory or Management Experience Preferred.
- Strong clinical assessment and leadership skills.
- Excellent verbal and written communication abilities.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Ability to respond to the needs of terminally ill patients and their families.
- Flexible schedule and participation in on-call rotation required.
Essential Duties And Responsibilities
- Coordinate and supervise the delivery of hospice services for patients and families.
- Ensure clinical services comply with hospice regulations, agency policies, and standards of care.
- Maintain accurate and compliant clinical documentation.
- Aid with patient admissions, certifications, recertifications, and discharges.
- Provide clinical guidance and support to nurses and interdisciplinary team members.
- Participate in quality assurance and performance improvement activities.
- Aid with staff orientation, training, supervision, and performance evaluations.
- Monitor clinician caseloads and scheduling.
- Coordinate care with physicians, the Medical Director, and interdisciplinary team members.
- Participate in administrative and nursing on-call responsibilities.
- Investigate and report patient concerns, grievances, incidents, and safety issues.
- Ensure compliance with federal, state, and accrediting agency requirements.
- Support community outreach and hospice marketing initiatives.
- Perform other duties as assigned by leadership.
Physical Requirements
- Ability to sit, stand, walk, bend, kneel, reach, and lift up to 50 pounds occasionally.
- Ability to travel to patient homes and other care settings as required.
- Must possess visual, hearing, and communication abilities necessary to perform nursing duties.
- May be exposed to infectious diseases, odors, disruptive patient behavior, and other healthcare-related environmental conditions.
