Qualifications

  • Active Registered Nurse (RN) license in the State of Georgia.
  • Hospice Experience Preferred.
  • Minimum of 3 years of nursing experience required.
  • Previous Supervisory or Management Experience Preferred.
  • Strong clinical assessment and leadership skills.
  • Excellent verbal and written communication abilities.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Ability to respond to the needs of terminally ill patients and their families.
  • Flexible schedule and participation in on-call rotation required.

Essential Duties And Responsibilities

  • Coordinate and supervise the delivery of hospice services for patients and families.
  • Ensure clinical services comply with hospice regulations, agency policies, and standards of care.
  • Maintain accurate and compliant clinical documentation.
  • Aid with patient admissions, certifications, recertifications, and discharges.
  • Provide clinical guidance and support to nurses and interdisciplinary team members.
  • Participate in quality assurance and performance improvement activities.
  • Aid with staff orientation, training, supervision, and performance evaluations.
  • Monitor clinician caseloads and scheduling.
  • Coordinate care with physicians, the Medical Director, and interdisciplinary team members.
  • Participate in administrative and nursing on-call responsibilities.
  • Investigate and report patient concerns, grievances, incidents, and safety issues.
  • Ensure compliance with federal, state, and accrediting agency requirements.
  • Support community outreach and hospice marketing initiatives.
  • Perform other duties as assigned by leadership.

Physical Requirements

  • Ability to sit, stand, walk, bend, kneel, reach, and lift up to 50 pounds occasionally.
  • Ability to travel to patient homes and other care settings as required.
  • Must possess visual, hearing, and communication abilities necessary to perform nursing duties.
  • May be exposed to infectious diseases, odors, disruptive patient behavior, and other healthcare-related environmental conditions.
