Remote Pharmacist
About the Role
As a Remote Pharmacist with Encompass Health, you will support multiple hospitals, each with a daily census of 30-60 patients. This role involves using ACEIT and various medication processes to assess, plan, intervene when necessary, implement, and evaluate patient care in compliance with each hospital's policies, protocols, and procedures.

Responsibilities
- Obtain census and confirmed admissions from Virtual Admission Board (VAB) for each facility.
- Use Microsoft Word and Excel to prepare basic documents and analysis.
- Access clinical systems for all hospitals serviced via remote connectivity.
- Demonstrate use of Powerchart and PharmNet to enter, verify, and manage medication orders.
- Use appropriate online resources to research and respond to drug information questions.
- Review system alerts for drug interactions, allergies, order & therapy duplications, overdose range, and renal dosing; follow appropriate processes to resolve issues.
- Document clinical interventions, including renal dosing, vancomycin dosing, and explanation of medication issues, determining if follow-up is necessary.
- Comply with individual hospital reporting requirements to communicate resolved and ongoing issues with hospital-based pharmacists.
- Demonstrate familiarity with formulary and non-formulary drugs at each hospital; provide cost-effective formulary medication recommendations when requested by physicians.
- Review TNF orders and convert to valid product orders when possible.
- Manage all generated protected health information in compliance with HIPAA privacy rules.
- Calculate appropriate doses and intervals for drugs requiring pharmacokinetic monitoring.
- Calculate and/or review creatinine clearance and recommend appropriate dosing based on renal function.
- Apply individual hospital policies, protocols, and procedures during documentation and verification of medications.
- Evaluate reported lab values to determine the appropriateness of medication therapy.
- Determine if the medication list provided with the patient is appropriate according to each hospital's protocol and accurately document transfer/discharge, home, and ER medication lists into Cerner in a timely manner.
- Identify problem issues and alert the verification pharmacist.
- Review controlled substance orders for appropriate dosing, duplication, and duration of therapy.
Requirements
- Licensed in good standing by the State Pharmacy Board for the state in which the hospital is located.
- Successful completion of a degree from an accredited pharmacy program or one determined to be substantially equivalent to a U.S. accredited program by an appropriate certifying agency.
- Minimum of 1 year of supervisory experience.
