Remote Intake Coordinator - Licensed Practical Nurse (LPN) - $21/hr - Day Shift

About the role

CareHarmony's Intake Coordinators (LPN) (NLC) (LVN) work comprehensively with providers to deliver value-based care management initiatives for their patients.

Responsibilities

• Accepting transfers from the Patient Enrollment team to conduct preliminary health assessments for newly enrolled patients in our network.
• Resolve patients' questions and create an open dialogue to understand needs.
• Identify and coordinate community resources with patients that would benefit their care.
• Provide patient education and health literacy on the management of chronic conditions.
• Affiliate with medication management, including identifying potential medication concerns, adherence, and coordinating refills.
• Affiliate with ensuring timely delivery of services to your patients; Home Health, DME, Home Infusion, and other critical needs.

Requirements

• Active Compact/Multi-State license (LPN) (LVN)
• Technical aptitude - Microsoft Office Suite
• Excellent written and verbal communication skills
• Must have active high-speed Wi-Fi
• Must have a home office or HIPAA-compliant workspace
