Job Summary
We are seeking a Remote House Calls Nurse Practitioner to support our in-home medical services program. This role provides virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes.
Responsibilities
- Conduct telehealth visits for patients enrolled in the House Calls program
- Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
- Perform care management needs assessments, follow-ups, and care plan adjustments
- Review patient histories, labs, imaging, and diagnostic results
- Support medication management and reconciliation
- Carefully coordinate care with primary care providers (if patients already have one), specialists, and care management teams
- Accurately document visits and clinical decisions in the EMR
- Ensure compliance with clinical protocols, quality standards, and regulatory requirements
- Participate in interdisciplinary case reviews and quality improvement initiatives
Requirements
- Active Nurse Practitioner (NP) license in the required state(s)
- National NP certification (ANCC or AANP)
- Minimum 2+ years of clinical experience as a Nurse Practitioner
- Experience in house calls, home health, geriatrics, primary care, or internal medicine preferred
- Prior telehealth or remote care experience preferred

Pay
The salary range for this position is $80,000 - $100,000 annually, commensurate with experience.
