Remote House Calls Nurse Practitioner

Job Summary

We are seeking a Remote House Calls Nurse Practitioner to support our in-home medical services program. This role provides virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program
• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
• Perform care management needs assessments, follow-ups, and care plan adjustments
• Review patient histories, labs, imaging, and diagnostic results
• Support medication management and reconciliation
• Carefully coordinate care with primary care providers (if patients already have one), specialists, and care management teams
• Accurately document visits and clinical decisions in the EMR
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements
• Participate in interdisciplinary case reviews and quality improvement initiatives

Requirements

• Active Nurse Practitioner (NP) license in the required state(s)
• National NP certification (ANCC or AANP)
• Minimum 2+ years of clinical experience as a Nurse Practitioner
• Experience in house calls, home health, geriatrics, primary care, or internal medicine preferred
• Prior telehealth or remote care experience preferred

Pay

The salary range for this position is $80,000 - $100,000 annually, commensurate with experience.

This is a full-time, remote position with flexible scheduling to accommodate patient needs and personal commitments.
