Starting Rate: $17/hour
Relief Direct Care Staff is responsible to supervise individuals' activities of daily living (ADLs), providing ongoing support, guidance, and role modeling on a consistent basis. Facilitate individual growth in the areas of personal responsibility, social skills, and community living skills. Promote community inclusion and self-advocacy.
Responsibilities
- Provide ongoing support, guidance and role modeling for individuals.
- Provide individual, informal counseling with individuals to discuss concerns and develop goals as needed.
- Assist residents with home management and community activities, self-advocacy, decision-making and empowerment, following goals appropriately.
- Assist residents with all ADLs, including showering, dressing, grooming, toileting, mobility, feeding, taking medications, and Range of Motion (ROM) exercises, both in the home and in the community as needed.
- Assist with weighing program participants, take vital signs, gathers and records other data as needed.
- Absolutely assist with teaching appropriate social and independent living skills and being a positive role model.
- Encourage program participants to perform tasks as independently as possible, assisting them to participate in all activities as appropriate.
- Attend and actively participate in resident, family, team and staff meetings and supervisions.
- Absolutely assist program participants with taking medications in accordance with MAP regulations.
- Absolutely accurately document observations and shift notes using computer database.
- Absolutely alert Program Management in the event of a program participant’s change in status or behavior.
- Absolutely complete all documentation and Incident Reports within program timelines.
- Absolutely ensure a clean, safe and home-like environment for clients.
- Absolutely remain alert at all times throughout scheduled work shift.
- Absolutely participate in core training and maintain current certification of required trainings (CPR/First Aid, crisis management, MAP) and maintain certification of required trainings.
- Absolutely provide crisis intervention and access emergency services as needed.
- Absolutely perform physical intervention in the event of a client crisis.
- Absolutely transport individuals using Advocates vehicles, including wheelchair lift-equipped vans as well as employee's personal vehicle as needed.
Qualifications
- A High School Diploma or equivalent degree required.
- Experience in providing direct care preferred.
- Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
- Must be able to perform each essential duty satisfactorily.
- Must possess strong computer skills.
- A high energy level and ability to function in a team atmosphere and independently.
- Ability to understand, speak, read and write fluent English or the language of the program assigned.
- Ability to communicate effectively both verbally and in writing and ability to use good judgment.
- Sensitivity to the needs of the individuals we support.
- Must have a valid driver's license and access to an operational and insured vehicle in addition to willingness to use it to transport residents.
