Job Summary

The Habilitation Coordinator works alongside a team of advocates to help adults with Intellectual and Developmental Disabilities (IDD) seeking admission to a Medicaid-certified nursing facility who may also have a co-occurring mental health diagnosis, ensuring access to and continuity of care. The Habilitation Coordinators' day-to-day activities follow the federally mandated Preadmission Screening and Resident Review (PASRR) program guidelines.

About the Role

Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment. Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Responsibilities

  • Apply a person-centered approach and perform a set of four service coordination elements: assessment, service planning/coordination, monitoring, and crisis prevention/management.
  • Work within a multidisciplinary team consisting of internal or external community partners.
  • Provide services that can be both office- and community-based and may require the use of a personal automobile.
  • Support individuals served who are diagnosed with IDD but may also present with co-occurring mental illness, substance use disorders, and/or chronic health conditions.

Requirements

  • Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.
  • Licenses: Current and valid driver's license and a clean driving record.
  • Experience: At least 2 years of experience providing services to individuals diagnosed with intellectual and/or developmental disabilities. Preferred Bilingual in Spanish.

Qualifications

  • Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safe care, and foster a supportive and welcoming environment.

Skills

  • Knowledge of state electronic systems such as CARE and TMHP.
  • Ability to facilitate transition duties when an individual requests to move out of the nursing facility.
  • Proficiency in using a personal automobile for interventions and services.

Pay

$24.18- $25.39 an hour based on education.

Schedule

Relief. Workdays: Monday - Friday. Workhours: 8:00AM - 5:00PM.
