Rehabilitation Nurse - Skilled Nursing Facility

Responsibilities

• Provide compassionate, high-quality care to seniors in a rehab setting
• Conduct assessments and develop individualized care and rehab plans
• Administer medications and treatments efficiently and accurately
• Coordinate with therapy and interdisciplinary teams to ensure safe recovery and optimal outcomes
• Educate residents and their families about care plans and health management
• Document all care activities accurately in the EMR system
• Support and collaborate with CNAs and other staff members
• Uphold the organization’s mission of dignity, respect, and person-centered care in a collaborative environment

Qualifications And Skills

• Valid registered nurse licensure
• Strong assessment, communication, and interpersonal skills
• Ability to work collaboratively within a multidisciplinary team
