Registered Nurse-Women's Health Clinical Navigator

About the role

The Veterans Affairs Oklahoma City Health Care System is seeking a Registered Nurse-Women's Health Clinical Navigator for Ambulatory Care to provide and/or supervise direct patient care. Assess, plan, implement, and evaluate delivery of care based on age specific components. Assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Responsibilities

• Provides patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing.
• Affords assistance to Veterans, family members, and caregivers in accessing the most appropriate services and options to meet complex healthcare needs.
• Coordinates care with multiple providers across all levels and sites of care, ensuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans.
• Oversees the patient’s journey through the diagnostic and treatment continuum, ensuring all steps in the process are scheduled, timely, and completed, while tracking the plan of care.
• Coordinates and facilitates, as needed, women's health care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow-up care.
• Interfaces with outside referral agencies and ensures their business and clinical practices follow regulatory standards.
• Anticipates and implements preventive patient care measures.
• Provides education, guidance, and support to Veterans and caregivers, while collaborating with interdisciplinary professionals across all levels of care.
• Notifies the Nurse Manager of any safety incidents, employee injuries, or concerns of team members not providing care according to the appropriate Standards of Care.
• Performs additional duties as assigned.

Requirements

• U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• English Language Proficiency: No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
• Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States, or the District of Columbia.

Qualifications

• Scope: Delivers fundamental, knowledge-based care to assigned clients while developing technical competencies.
• Education: An Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required.
• Experience: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States, or the District of Columbia.
