About the role
CommuniCare+OLE is a federally-qualified health center network offering comprehensive care across Napa, Solano, and Yolo Counties. It serves over 70,000 individuals, providing services including medical, dental, behavioral health, and substance use treatment, among others.
Responsibilities
- Works as part of a team with the Health Center Manager, medical providers, and clinic support staff to provide quality, coordinated care to patients.
- Provides clinician support, patient case management, patient education, triage, monitoring of patient flow, and indirect nursing supervision of the medical assistants.
- Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within scope of practice.
- Provides direct one-on-one patient education.
- Performs independent face-to-face or telephone triage using written triage protocols.
- Accepts and correctly transcribes verbal or telephone orders from clinicians.
- Provides follow-up to patients recalled for abnormal labs/tests.
- Transmits new, updated and refilled prescription orders to pharmacies on behalf of prescribing providers.
- Performs diagnostic and laboratory tests including but not limited to audiometry, EKG, urine dips, accu-check, urine HCG, hemocue hemoglobin, etc.
- Processes clinical patient information for providers in patient electronic health records (EHR).
- Affords assistance in meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
- Assists with coordinating care to other CommuniCare+OLE services, including dental care, behavioral health, diabetes education, etc.
Requirements
- Accredited RN program or higher required.
- Current unencumbered California Registered Nurse license required.
- Current BLS/CPR certification required.
- Previous RN Work Experience In a Healthcare Setting preferred.
Skills, Training
- Strong Analytical And Problem-solving Skills Required.
- Strong interpersonal and customer service skills required; must have ability to work with a diverse population.
- Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
- Strong computer and data entry skills required; electronic health record system preferred.
