Responsibilities
- Provide care and services for a group of patients, visiting them where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities.
- Serve as a contributing member of the interdisciplinary team, providing expertise regarding psycho-social assessments, interventions, and knowledge of community resources.
- Work in either a Case Management, Admissions, or Resource role.
- Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership in a team environment.
- Work closely with patients on a one-to-one level while receiving strong support from a patient care team.
- Manage independent day-to-day activities including phone, email, and written communication, meetings with healthcare professionals, and understanding hospice rules and regulations.
- Utilize time management skills to manage visits, documentation, meetings, and respond to urgent needs by prioritizing and triaging multiple concerns.
- Learn and use an electronic medical records system.
- Drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
Requirements
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
