Pay

Attractive pay range of $50.00 to $60.00 per hour.

Responsibilities

  • May be assigned RN visits as a member of the care team and/or may supervise a patient caseload, including LPNs and HHAs.
  • Performs patient comprehensive assessments and collaborates with the multidisciplinary care team to develop, modify, and implement an individualized plan of care as per physician orders, ensuring quality, proper discharge planning, and desired outcomes.
  • Facilitates interdisciplinary communication and follow-up through care coordination for assigned patients or caseload for the duration of service.
  • Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
  • Participates in clinical outcomes monitoring, follow-up, and care center performance improvement initiatives.
  • Initiates, plans, and provides staff education; performs competency and skills validation for staff as needed.
  • Utilizes accepted company teaching materials for patient and family education.
  • Regularly communicates patient progress to the clinical manager.
  • Completes documentation timely, accurately, and at the point of care, following industry standards, conventions, and guidelines (e.g., OASIS assessments, SOC/admissions, recertifications, ABNs, subsequent visits, physician orders, care coordination).
  • Other duties as assigned.

Requirements

  • One year of nursing experience.
  • Current RN license specific to the state(s) of assignment.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and liability insurance.
