About the role

Lead coordination of multidisciplinary care in a stepdown unit, serving as the central contact to ensure seamless patient services.

Responsibilities

• Implement and evaluate patient-centered care plans to optimize outcomes and recovery.
• Accurately document patient care journeys and communicate effectively with the care team.
• Assess and facilitate discharge planning to support smooth transitions to post-acute settings.

Qualifications & Skills

• Registered Nurse licensure from the Wisconsin Board of Nursing or a multi-state/Compact State license.
• BLS Provider certification within 30 days of hire, accepted from AHA or ARC.
• Diploma from an accredited nursing program or equivalent professional licensure.
