About the role

Care For the Homeless (CFH) is a nationally recognized nonprofit leading the way in caring for those experiencing homelessness and housing instability in New York City. Since 1985, as the city’s first “Health Care for the Homeless” program, we have integrated healthcare, shelter, and social services in a coordinated, innovative model of care that meets people where they are. The RN is part of a multi-disciplinary team that provides clinical support to medical and behavioral health practitioners, in accordance with Article 28 and Patient-Centered Medical Home (PCMH) standards, following the principles of Trauma-Informed Care.

Responsibilities

  • Review patient resource schedules and organize patient flow.
  • Accompany patients to exam/procedure rooms, assisting with walking, transferring, dressing, administering medication, treatment, and/or vaccines.
  • Complete patient intake/screening, provide prepared patient education, and discharge patients.
  • Retrieve laboratory and radiology reports for medical or behavioral health practitioners to review.
  • Obtain and accurately document patient’s family and social history, vital signs (blood pressure, pulse, temperature, pain), height, weight, oxygen saturation, and body mass index (BMI).
  • Perform basic audio and visual screening.
  • Administer vaccines, medications, or treatments and fulfill specific orders as indicated by the Practitioner (e.g., intramuscular injections).
  • Perform point-of-care testing (e.g., urinalysis, rapid strep, rapid flu test, INR, phlebotomy, urine pregnancy test, glucose testing).
  • Remove sutures or staples under supervision, provided the patient is evaluated before the procedure.
  • Assist Practitioner with examinations and procedures.
  • Prepare and organize exam room supplies, ensuring equipment and supplies are readily available and not expired.
  • Maintain emergency bags, oxygen tanks, refrigerator logs, and other clinical equipment per CFH protocols.
  • Maintain supply inventory, order medications, immunizations, and other supplies; maintain accurate records of controlled substances.
  • Clean, disinfect, and pack instruments for sterilization; maintain sterilization logs.
  • Perform duties in accordance with Department of Health (DOH) guidelines, OSHA blood-borne pathogens standards, National Patient Safety Goals, and CFH policies.
  • Perform clerical functions, including patient registration, appointment scheduling, verifying insurance, updating PCP, applying sliding fee scales, answering phones, relaying messages, completing medical forms, and scanning documents into the EMR.

Requirements

  • Current NYS Registered Nurse licensure.
  • Basic Life Support (BLS) Certification.
  • Minimum one (1) year of experience preferred.
  • Experience in ambulatory care, emergency room, and/or physician office settings preferred.

Qualifications

  • Bachelor of Science in Nursing (BSN) preferred.
  • Prior eClinicalWorks (eCW) experience preferred.

Compensation is based on experience.
