About the role
The Admissions Nurse’s primary focus is to perform the initial SOC (Start of Care) visit and other visit types as assigned by the agency. Responsibilities include determining the appropriateness for home care, conducting an overall assessment, medication reconciliation, developing the plan of care, and providing direct care and treatments as ordered. Documentation must meet quality standards and reflect nursing care, treatments, and patient responses.
Responsibilities
- Determine appropriateness of patient for home care
- Develop patient-centered plan of care based on comprehensive assessment
- Coordinate with internal and external resources to meet patient needs
- Communicate client care issues to interdisciplinary team including physician
- Coordinate services and identify needs for additional services
- Report findings and plans to designated RN Case Manager/Care Team and/or Clinical Supervisor
- Supervise home health aides in performing tasks related to patient care
- Comply with Nascentia policies and procedures and quality initiatives
- Maintain confidentiality and demonstrate knowledge of infection control practices
- Represent self and agency in a professional manner
Requirements
- Current New York State registered professional nurse license
- Current and valid driver’s license and auto insurance, and a vehicle suitable for work
- Self-directed and organized with the ability to work independently
- Good oral and written communication skills
- OASIS Certification or willingness to obtain within 1 year of hire
- Minimum one year experience in Home Health Care
- Physical abilities to sit or stand for long periods, perform physical activities, and drive in various weather conditions

$75,250 - $87,138.92 annually
