Registered Nurse - Skilled Nursing - Nights (UTICA, NY)

About the Role

Assesses, plans, implements, and evaluates the nursing care of residents within the skilled nursing setting. Ensures continuity of care between shifts by providing direct care and supervising CNAs, LPNs, and supportive staff. Participates in resident and family teaching/conferences and acts as a resident advocate aligned with the facility’s mission.

Responsibilities

• Assesses residents on admission, when conditions change, and documents findings in Care Plans and Medical Records.
• Educates LPNs on PPS documentation requirements and ensures compliance.
• Assesses and reassesses pain using appropriate pain management techniques; educates residents and families on pain management.
• Reviews records to ensure pain logs are complete.
• Sets nursing goals and prescribes actions based on resident assessments.
• Performs resident care in a safe environment, minimizing medical/healthcare errors.
• Submits timely, clear, and concise documentation; notifies physicians of changes in resident conditions and follows through on actions.
• Carries out physician’s orders promptly and accompanies physicians on floor rounds.
• Prepares 30- and 60-day reviews in advance.
• Provides thorough shift reports via walking rounds and follows up on concerns from residents or families.
• Documents and investigates complaints, develops solutions, and keeps leadership informed.
• Implements and evaluates the resident’s plan of care based on interdisciplinary team assessments.
• Treats residents and families with respect, addressing psychosocial, cultural, ethnic, and spiritual needs.
• Administers medications following the five rights of medication administration to reduce errors.
• Interprets clinical changes in relation to medical diagnoses.
• Takes responsibility for professional growth and learning new clinical skills independently.
• Supervises and delegates tasks to LPNs and CNAs to achieve excellence in nursing care.
• Acts as a preceptor/resource person for other team members.
• Protects resident rights as an advocate.
• Remains calm in emergencies and complies with regulations and facility policies.
• Performs other duties as required.

Requirements

• Graduate of an accredited nursing program.
• Good interpersonal, written, and verbal communication skills.
• Registered Nurse licensure in New York State.

Preferred Qualifications

• Bachelor of Science in Nursing or higher degree.
• One year of experience in long-term care.
• Basic Life Support Certification.

Pay

$27.00 - $42.00 per hour.

• Hours: 8-hour shifts
• FTE: 0.94 (Full-time, non-union)
