Registered Nurse (RN) - Weekend Shift - Home Health

Responsibilities

• Provide skilled, patient-centered care in patients' homes to support recovery, chronic condition management, and independence.
• Perform comprehensive assessments, develop and adjust individualized care plans, and monitor patient conditions.
• Communicate changes to physicians promptly, educate patients and caregivers, and coordinate with interdisciplinary teams.
• Ensure safe discharge preparation and document care accurately in electronic medical records.
• Deliver compassionate, quality care that improves long-term outcomes.
• Partner with physicians and team members to support safe patient recovery at home.
• Educate patients and families on treatments, medications, and safety.
• Coordinate care seamlessly with other providers to ensure compliance and safety.

Requirements

• Graduate of an accredited RN program and active South Carolina RN license or Compact License.
• Minimum of one year of nursing experience; familiarity with home health, medical-surgical, critical care, or case management preferred.
• Experience with OASIS documentation is a plus.
