About the role

Join the PruittHealth Hospice family as a Registered Nurse (RN) Case Manager providing weekend on-call hospice services (Friday 4:30 PM - Monday 8:30 AM). This role involves delivering specialized hospice care in the patient’s home or care facility as part of a uniquely specialized team of caregivers.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Ensure all documentation and records are completed in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.

Requirements

• Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.

Schedule

Full-time, weekend on-call shift: Friday 4:30 PM - Monday 8:30 AM.
