About the role

Accountable for ensuring patient care is coordinated and managed appropriately. Focuses on the work of the department, quality monitoring, teaching and serving as an ongoing resource.

Responsibilities

  • Demonstrates ability to plan and be responsible for the day to day operations of the assigned agency.
  • Receives case referrals and reviews available patient information related to case, to determine home care needs.
  • Sets up appropriate personnel to case as needed.
  • Attends care conference meetings with hospice/home health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Oversees the implementation and evaluation of in-service and continuing education programs for staff.
  • Complies with accepted professional standards and principles.
  • Serves as a liaison for Avera developing partnerships with clinics, physicians, nursing home, assisted living and other health care team members.
  • Participates actively in performance improvement teams and activities.
  • Provides input and assures compliance with departmental policies and procedures, scope of care, safety, environmental and infection control standards.
  • Promotes and maintain as environment that is in compliance with State, federal and regulatory requirements.
  • Provides input for budget preparation, monitors productivity and expenses, and develops plan of action to correct variances.
  • Provides direct clinical patient care.
  • Provides direct support to any agency branches of alternative site locations on a daily basis.
  • Responsible for interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.

Qualifications

  • The individual must be able to work the hours specified.
  • To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.
  • This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier.
  • Upon Hire: Registered Nurse (RN) - Board of Nursing.
  • Active license in the state of practice.
  • 1-3 years Training or experience in health services administration with at least 1 year of supervisory or administrative experience in home health care or related health programs.
  • Preferred: 1-3 years Hospice nursing experience.
