Registered Nurse (RN) – SOC & Visit Nurse (Home Care) Flexible Schedule | Immediate Openings BONITA SPRINGS 34135 (SPANISH SPEAKING))

Responsibilities

• Complete Start of Care (SOC) assessments in accordance with state and agency guidelines.
• Perform Skilled Nursing Visits including wound care, medication management, patient education, and chronic disease monitoring.
• Develop and update individualized care plans.
• Communicate with physicians, case managers, and the interdisciplinary team.
• Document accurately and timely in the EMR.
• Provide high-quality, compassionate care to patients in their homes.

Requirements

• Active Florida RN license.
• One year of nursing experience (home health preferred but not required).
• Valid driver’s license and reliable transportation.
• CPR certification.
• Level 2 background screening (or willingness to obtain).
• Strong clinical assessment skills.
• Ability to work independently.
