Responsibilities
- Resident assessments
- Care plan development by collaborating with other healthcare professionals
- Coordination with IDT to optimize health outcomes
- Supervision of CNAs
- Compliance with IDPH/CMS
Requirements
- Active Illinois Registered Nurse license
- Long Term Care (LTC) experience preferred
- Strong, effective communication
- Exemplary leadership skills
