About the role

Provides direct nursing care including assessment, plan of care, education, and evaluation for assigned patients. Performs patient duties necessary to maintain the flow of patient care as well as coordinates the delivery of care provided by other care team members and disciplines.

Responsibilities

• Assesses patients’ physical, psychological, social, spiritual, and developmental needs.
• Develops and implements individualized care plans based on assessment findings.
• Evaluates patient progress and modifies care plans as needed.
• Conducts patient education sessions to promote health and wellness.
• Coordinates care with other healthcare professionals and support staff.
• Maintains accurate and timely documentation of patient care activities.

Requirements

• Current California RN license (BSN/MSN preferred).
• Evidence of continuing education.
• Prior experience in an acute care facility or formal classroom education.
• Current Basic Life Support Card.

Qualifications

• Written and verbal communications, interpersonal skills.
• Basic organizational skills.
• Basic computer skills.
• Demonstrated leadership abilities.
• Knowledge of principles of patient/family teaching.
