About the Role
Our Registered Nurse (RN) will perform a variety of duties to support the company’s continuity of care. This role includes conducting all assigned visits within the scope of practice, such as OASIS visits, skilled nursing visits, and supervised visits. The RN will also participate in the compensated orientation of new hires and administer skilled nursing care for patients of all ages in their place of residence. Responsibilities include coordinating care with the interdisciplinary team, patients, families, and referring agencies, as well as assuming responsibility for care coordination.
Responsibilities
- Provide case management and conduct supervised visits.
- Perform skilled and comprehensive assessments of patients during initial admission.
- Assess and document patient conditions at each visit, including vital signs.
- Deliver skilled nursing care as outlined in the plan of care, such as administering medication, cleaning/dressing wounds, and assessing healing progress.
- Educate patients on home health care procedures and strategies.
- Maintain the overall clinical record, including visit notes (completed within 24 hours), comprehensive assessments (including OASIS, completed within 48 hours), medication profiles, and timely submission of orders/plans.
- Ensure confidentiality of patient PHI in compliance with HIPAA laws.
- Provide palliative care to keep patients comfortable.
Requirements
- Current Texas State license as a Registered Nurse.
- 2+ years of experience as an RN in Home Health.
- Current CPR certification.
- Reliable transportation, Texas Driver’s License, and auto liability insurance.
- Current TB Skin Test.
- EMR experience (Kinnser preferred).
