Registered Nurse (RN) PRN Case Manager
About the role
The RN Case Manager is a hybrid position supporting patients and family members in both the inpatient setting and the emergency department. This role involves evaluating appropriateness of medical care, participating in discharge planning, coordinating patient transfers, and providing discharge education.
Responsibilities
- Maintains patient and family satisfaction by monitoring their feedback and addressing concerns.
- Coordinates discharge planning, including patient transfers to other facilities and referrals to community resources.
- Documents case management activities, plans, and outcomes accurately.
- Communicates effectively with providers, payors, and ancillary services to facilitate timely care progression.
Requirements
- Current Arizona or Compact State RN license.
- Associate's Degree in Nursing (ADN) from an accredited nursing school.
- Bachelor's or Master's Degree in Nursing preferred.
- Basic Life Support (BLS) certification within 30 days of hire.
- Accredited Case Manager (ACM) certification within 2 years of hire preferred.
- 2 years of RN experience required.
- 2 years of Case Management experience preferred.
Qualifications
Applicants must have a current Arizona or Compact State RN license and an Associate's Degree in Nursing (ADN) from an accredited nursing school. Additional qualifications include a Bachelor's or Master's Degree in Nursing, Basic Life Support (BLS) certification within 30 days of hire, and Accredited Case Manager (ACM) certification within 2 years of hire. Two years of RN experience and two years of Case Management experience are also required.
