Registered Nurse (RN) - PRN

Role Overview The MDS Nurse is responsible for organizing, completing, and evaluating resident assessments and care plans in compliance with state and federal regulations. They coordinate the MDS, CAAS, and care planning processes upon admission, ensuring timely updates and documentation. The role involves observing and reporting changes in residents' conditions, establishing assessment reference dates, and collaborating with physicians and the interdisciplinary team. The nurse participates in quality improvement initiatives, community surveys, and investigations of resident concerns. They mentor other MDS staff, develop training, and promote resident dignity and participation in care planning. Administrative duties include completing medical forms and reports, and maintaining a professional, caring environment. The position requires a nursing degree, a current unrestricted LPN or RN license, and at least two years of MDS experience. Strong communication skills and proactive care are essential. No travel involved.
