About the role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Responsibilities
- Carry a high-quality caseload: Ensure visit quality, with OASIS accuracy that holds up to review.
- Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
- Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
- Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
Requirements
- Active Michigan RN license
- Reliable transportation, valid driver's license, auto insurance
- Home health / OASIS experience strongly preferred
Qualifications
- Dedicated clinician: home health experience strongly preferred
- High ownership: you treat your caseload like your own panel and your pod like your own business
- Open mindset: Energized by the opportunity to shape the playbook and refine SOPs
